I wanted to thank you for everything that you have done for me with my recovering process. As you know, on April 12, 2008, I sustained a grade three concussion, due to a fall off the horizontal bar in a gymnastics competition. After the fall I was unconscious for about ten to fifteen seconds, when I regained consciousness I could not stand up or walk on my own. I was rushed to Leigh Valley Hospital by ambulance, where they ran CT scan on both my head and neck, they found no damage and discharged me later that night with a concussion. Later I was diagnosed with post-concussion syndrome, which only brought further frustration and agony. I would have headaches everyday and couldn’t do normal activities such as watch T.V, read, concentrate, and was extremely photophobic and phonophobic. I went to three different neurologists and a neuropsychologist, where I went through many different tests, which brought a lot of pain and discomfort, and was put on many different medications.
Eventually, all of the doctors told me that the only thing that I could do was to give it time and wait. That was the hardest thing for me, having to wait without knowing when or if I would ever start feeling normal again and be able to do all the things that I used to. The last neurologist I saw told me that all of the pain and discomfort I was going through with the headaches couldn’t damage my brain, and I would just have to tolerate them. Once I heard that, I accepted that these headaches may or may not go away, and I was prepared to live my life putting up and managing the headaches.
Thankfully, I am a huge flyers fan and saw what happened with Simon, and how he was able to make his recovery. So naturally my family and I wanted to look into some of the help and procedures that he received. Then I had my appointment with you, and it completely changed my life around. Now I’m feeling great, I am no longer sensitive to light and noise, I can read on my own again, I can concentrate longer now, and I can do normal day activities without a problem now, but most importantly my headaches are little to none, and it gets better and better with every treatment. Due to how great I’m feeling physically, it’s also making me feel so much better emotionally as well. I’m feeling more confident, smiling and laughing a lot more, I’m starting to feel like my old self again, and I believe that I’m going to make a full recovery and be able to do everything that I was able to before the accident, and I have you to thank for that. I am writing to highly recommend the services of Dr. Greenberg. This is my story. I suffered from chronic pain due to an auto accident six years ago. I was diagnosed with injuries to my brachial plexus and low back. I followed the recommendation of my doctor and received conventional medical treatment. This involved many visits to the chiropractor, physical therapists, neurologists, pain management doctors, and other specialists. I had X-rays, MRIs, and CAT scans done. Numerous treatments were performed on me, including chiropractic adjustments, TENS unit, hot and cold therapy, cortisone shots, spinal blocks, and many others. I was your patient until about 3 1/2 years ago when you healed both of my knees (ACLs, PCLs, MCLs) from major damage due to a skiing accident in which I blew out both knees. I just thought it would be nice to give you some follow-up comments as to how your work held up. As you may recall, you did give me the green light to ski again, a prospect that seemed impossible the prior year. That year I did ski in Vermont and did well. The following two years, I skied in Colorado, each trip having five straight days of skiing. I did well and beat all of my buddies who are about 25 years younger than me (some of it is technique of course since youth will trump age) but I skied all 5 consecutive days on each trip, all day. This year, in which I am turning 69 at the end of April, I go to the gym (amongst other things) and do interval training on a treadmill. I run in spurts of 8 miles an hour, sometimes ratcheting it up to 9 miles an hour. Still going strong and the knees are holding up well.

Gail Silver consulted Dr. Magaziner after suffering from ankle and foot pain as a result of torn ligaments. Her orthopedic surgeon previously told her she would have to undergo surgery, but after just two prolotherapy treatments, all her pain was gone, making an operation unnecessary. Here’s what Gail had to say…
“I knew I had really hurt my foot by the ankle bone when after two months of waiting for it to heal by itself, the pain became exacerbated to the point that I could not step, walk, or stand. I usually wait for my body to heal itself. I made an appointment with an orthopedic doctor who had helped me before with my heel. He examined my foot, took x-rays, and told me I had stretched the ligament, and hoped I had not torn them. The surgeon gave me a brace to wear for two months. But instead of getting better the condition got worse. I tried to work as a substitute teacher, hobbling and in pain. When I went back to the doctor, he took an MRI and said I had torn ligaments and needed an operation. That would involve either having to take a piece of my heel bone or calf, or attaching the ligament to another ligament. He said healing would take 18 months and I would need help at home, being totally bedridden for the first two months and then Ineeding to use crutches and a walker. I would also require rehabilitation and physical therapy. He sent me a pamphlet explaining everything. It said the surgery could not guarantee that I would be pain free.
Up to this point, I had never heard of prolotherapy. Dr. Magaziner had treated my allergies for years. So I called him, told him my problem with my ligaments, and asked if he could help. He thought he could, and I had all of the reports sent to his office. By the first treatment, I was in so much pain that I never noticed any pain from the prolotherapy itself. Within four days, I could put my foot on the ground and walk with no more limping. Each day it got a little better. The next month, I got my second treatment. The foot did swell for the next three days and was slightly sore. But by the following week I was fine. I was even able to walk all day in New York City on concrete streets. It was like a miracle! No operations for me! It is a crime that surgeons and standard doctors do not look into alternative procedures such as this.”

I am forwarding the Web site and information about Dr. Scott Greenberg, my favorite doctor in the whole wide world. He is the doctor that healed my knees using a revolutionary, non-invasive treatment called prolotherapy after the car accident I had in 2003. He also introduced me to the Cookie Diet, which made it possible for me to shed 65 pounds of excess, energy-draining weight and return to my normal slender self.
Now I know many of you have been dealing with pains and pangs in various areas of your anatomy -- both internally and externally -- and have run the gamut of treatments, most of which have only been either of the stop-gap, band-aid, or non-effective variety. Thank goodness Scott Greenberg is not that kind of doctor -- and he leaves nothing to chance. He will not leave you ill and will not scam you, but will let you know up front whether he can or can't help you (and thank goodness he's got a 98% success rate). Anyway, after he worked on my knees and straightened out my spine; after making sure that everything else was in order, and completing the first phase of prolotherapy, he informed me that my knees would be fine. How can I start to thank you for repairing/healing my right rotator cuff tear. At first, you weren’t too sure because of the excruciating pain I was in last September – I also had my doubts because I had a full thickness tear with retraction. "My husband suffered with a painful knee for months. We were away for the winter when he finally visited an orthopedist who ran an MRI which determined a torn meniscus. He was scheduled for an operation, but had to get a medical clearance. He couldn't get a medical clearance until he got his blood pressure down.
They started him on medication, and the pressure went up. They increase the medication and the pressure got higher. It was time for us to come home,so he went to an orthopedist at home, who made the same diagnosis. He needed an operation, but the blood pressure was still an issue. They tried a different prescription medication. He developed a rash. The whole thing made no sense to me. He was getting to the point where he could no longer walk.
I did quite a bit of research on what they meant by meniscus repair and felt he was mislead. During the course of my research, I found prolotherapy as an alternative treatment. To "shut me up" he visited Dr. Greenberg. He decided to give it a try. Dr. Greenberg had told him that he would need several treatments. He also started alternative medicine for his blood pressure. "Imagine yourself lying in bed with your eyes closed, unable to read, to talk, to communicate Imagine yourself crawling on the floor of your house and asking your family to bring your meal to your corner where you can chew a few pieces with your eyes closed because every single movement of your jaws makes your dizziness even worse. And the pain - a constant, persistent, unbearable pain in your neck, head, eyes as if you're wearing crown of the thorns. Not even mentioning panic attacks in the morning when you lose your reason because the world you're living in is completely unbalanced, shaky, painful.
When I was diagnosed with Barre-Lieou syndrome my doctor told me that this was incurable and that I had to learn how to live with it. She recommended to me to order prismatic glasses that would allow me to read and write without moving my eyes. She also sent me to a physiotherapist for positional exercises, which were supposed to decrease my dizziness. After limping and dragging my left leg for a couple of months due to a painful Torn Meniscus (diagnosed by MRI) of my left knee I had enough. I was experiencing difficulties going up an down the stairs to the point that my husband and I began to think about selling our two story house and purchasing a ranch house instead. Going down the stairs was the worst. I would take one step at a time, with the left leg leading, so as not to bend the knee for the pain would be excruciating. Since 1944 my right knee has been troublesome. After the removal of a piece of cartilage from the knee, I no longer had the support to sustain my weight for walking or climbing stairs. The orthopedic surgeons said nothing could be done to improve my condition, other than a surgically fitted knee brace. I opted for the stretch knee brace when the pain and inability to function normally required some temporary relief. Years later the knee deterioration worsened. The suggestion for a knee implant left me cold and unresponsive.
My chelation treatments require a periodic check-up of my overall health. On a hot summer day, I got lucky! After checking my chart preliminary to examination, Dr. Greenberg saw the long scar on my right knee. "What's that?" was the inquiry. Then, I recited what was mentioned in the foregoing portion of this letter. Dr. Greenberg advised that prolotherapy would help. By injecting medication into the knee, the subsequent irritation would cause the body to generate new tissue to reinforce the desperately stretched and torn ligaments, providing the needed support I required.
After three treatments (administered three weeks apart), I felt improved response and strength in my right knee. This last visit to the medical center for the next treatment, I went up the stairs without hanging onto the bannister. I live in a two level townhouse. My usual ascending to the second floor necessitated hanging onto the bannister and pulling myself up the stairs by arm power. No longer do I have to struggle to go up stairs! I had been battling sciatica for about 2 years. I was going to a chiropractor for 1 and a ½ years and every time I had an attack, he was able to help me, except for the last time. Treatment of knee problems
I was suffering with a knee problem for over a year and it was getting worse each day. During one of my visits to Dr. Greenberg he noticed I had great difficulty both bending my knee and trying to walk. Dr. Greenberg informed me of a new treatment that was available and had been successful with many of his patients.
After an examination of my knee, he recommended the prolotherapy treatment. Now, sixty (60) days after three 3) injections later, my knee is 90% better. I am now able to walk, bend, drive and have a restful nights sleep and no longer have the severe pain I was experiencing over the past year.

Several years ago I suffered sports injuries, one to my left knee which made my knee feel wobbly and tender if I put any stress on it, and the other injury to my right shoulder. The doctors at the hospital said I had strained all the ligaments in that shoulder. Due to that shoulder injury my range of motion was severely restricted and I had soreness whenever I exercised that arm or played golf. In addition, I developed tennis, and golf elbow in my right elbow and forearm area. Whenever I played sports, I would wear an ace bandage on my left knee and sometimes on my right elbow, and I would experience soreness or stiffness afterward. After receiving just two to three prolotherapy treatments five or six weeks, I don't need the ace bandages on my knee or elbow. My range of motion has improved greatly in my right shoulder, and I have no more soreness either during or after playing golf. When I first came to see Dr. Greenberg, my knee had pain every time I took a step. Walking up or down steps was very painful. I depended on the railing to go up or down. Now after 3 treatments of Prolotherapy, I can walk up or down without holding on to the railing. Now I am pain free after a year of pain and limping.

You will be gratified to learn that the series of injections you gave me for my plantar fascitis have been very successful. After the first three or four, I reached a plateau, and did not see much further improvement. However, after the last shot in June, the change was miraculous! In August, we spent four days in Montreal, walking all day every day. I never had any pain whatsoever. Last week, we spent three days at the shore, and I took great pleasure in walking every morning along the boardwalk.
For the last month, I have been back to my regular exercise, walking ¾ hour, three mornings a week. As you can see, I have much to thank you for.

I can attest to the fact that for 3 or 4 months I experienced a great deal of pain in my neck area (literally) - more concentrated in the right area. Chronic knee pain
During Feb. 2000 I met with Dr. Greenberg for the first time. My original visit was in regards to my blood pressure. During the examination I was asked, by Dr. Greenberg, if I had any other problems. I answered that I had continuing pain the both knees (probably the result of athletic injuries). Dr. Greenberg asked me, "what if I could help with the knees without operating on them, or using cortisone or steroid shots?" My response, "that would be wonderful!" In April we started a series of prolotherapy injections.
After four or five treatments my knees were so much better I could not believe it. All of the pain is gone from the right knee and practically all of the pain is gone from the left knee. I can walk up and down stairs with ease. Walking in general has improved and I am no longer worried about my knees buckling. My good fortune continues. I am so happy that I met Dr. Greenberg who has been in the forefront of prolotherapy practice.

I had been experiencing pain in my lower back across my buttock and down my left leg. The pain was so bad at times I would lose my balance. I fell as though I have a high tolerance for pain and I am not a big believer in medicines especially painkillers. I asked my Doctor to send me for an MRI. I hoped the answer would be there and it wasn't. I was told it was not bad enough to operate on and I was glad because that was my last choice and was told it would go away as fast as it came on. The pain that is. I had been going through this for eight months and that was a little much for any pain, especially this type which was my back, my buttocks, my leg and my mental being. It was at this point I was recommended to see Dr. Greenberg, whom made me feel very comfortable at our first meeting. I felt confident with his confidence in what he was going to do to solve my problem.
The first day after the first treatment I questioned myself as to why I did this and found out it was part of the program and each day. In June of 2000 I injured my back, and was in extreme pain for many weeks. I went to my family doctor, he said I had a sprain in my lumbar region. He gave me steroids and lots of pain medicine. I started going to physical therapy, and a chiropractor but the pain would not go away. An MRI was done, I was told I have disc herniation L 4-5, also degenerative disc disease. I then went to an Orthopedic, she told me I have a pinched nerve. It sends pain across my back into my right leg. She gave me more pain medicine. A friend of mine told me how Dr. Greenberg helped her foot become pain free after her operation. So I decided to try Dr. Greenberg.
I started in Oct. with the prolotherapy treatments. After an amazing 3 visits I no longer had my lower back pain or the pain down my leg! I am a firm believer the prolotherapy works, and I am happy to be pain free. Dear Dr. Greenberg,
At age 14 I was struck by a car, damaging most of my ligaments and tendons in my right leg. I am 43 now and my right knee was getting horrific pain for the last 3 years as it increased over the years. The pain was so bad I couldn't climb stairs using this leg, squat, jump or even touch my kneecap. The leg also gave out a few times a day while walking.
Two orthopedic surgeons agreed, there wasn't any treatment or surgery to repair ligaments and tendons in my case. They prescribed painkillers and anti-inflammatory pills and told me to stop playing basketball. Then I saw Dr. Greenberg on T.V. and made an app.. After 3 prolotherapy sessions, I am 100% pain free. The strength and movements in the repaired knee is the same as my other knee.
People are surprised when they see me run upstairs, jump and squat. Thank you, Dr. Greenberg & Staff. In fact, although I live 1 and ½ hours away I would visit them for any other medical problem I incur in the future. Regardless of the medical problem, I tell all my friends about it.

Pain free versus 'pain managed'
To Whom It May Concern:
This letter constitutes the filing of an appeal as a response to your claim decision reflected in your December 13, 2005 communication to me regarding my services from Dr. Scott Greenberg from 6/13/05 to 10/19/05. I am a registered nurse with a history of increasing chronic lower back and radiculopathy of the right leg over the last 3 ½ years. I was assessed over that period of time by several primary care physicians, two orthopedic surgeons and two neurosurgeons. Extensive diagnostic testing did not determine exactly what caused my severe pain. I was told that I was not a surgical candidate. I was put on Vioxx (!!!), Celebrex and several other anti-inflammatories, none of which decreased the pain. I saw two chiropractors for an extended series of their therapies and was also referred by physicians to physical therapy for three series of physical therapy (not three sessions, but three extended series).
None of these therapies decreased the pain, in fact, each time, the physical therapy and chiropractic therapies exacerbated the pain! Twice I was sent to Pain Management. The physician at the first pain management facility gave me a prescription for (Vioxx!!!) The second time I was referred to Pain Management (at Lancaster General Hospital in Lancaster, PA) I had to wait two months JUST to GET an appointment for a physical assessment (due to high demand for Pain Management!!!)
Then I waited another several months to get a Cortisone shot. On March 9th of 2005, I was given a cortisone injection in the right sacroiliac area by a Dr. Chen. Radiology was there with their fluoroscopy unit. I received ONE injection, with the fluoroscopy unit attended by two radiology technicians, and as I said before, this was done at the Lancaster General Hospital for a total charge to my insurance company (Wausau) of $1410.00 !!!!! Of course, there was an amount of contractual write-off, but Wausau promptly paid the amount of $883.36 for one shot, one appointment!!! I was told by Dr. Chen to wait several weeks to see if there was any diminishment of my pain. THERE WAS NO DIMINISHMENT OF PAIN!!!
At that time, I was then exasperated at traditional approaches to pain, not to speak of the poor stewardship of funds that I and my insurance company were using to GET NOWHERE in this attempt to identify and heal my pain! You have to know that although I am 60 years old, I am in excellent health, weigh 118 pounds and I am on no medications. I have always exercised (until not able to with the increasing pain) and am VERY conscientious regarding health and nutrition. Prior to my pain, I was very active, and in fact was the Director of Health of a local college (which at times required a significant amount of physical stamina). My frustration with traditional medicine's inability to resolve my back and leg pain and the ensuing poorer quality of life that I was experiencing led me to do research on my own regarding other approaches to pain therapy.
I became aware of Dr. Scott Greenberg in Cherry Hill, NJ and inquired about the nature of prolotherapy. Through information on his practice's website and also extensive other research, I decided to pursue this approach. Although I was traditionally trained in medicine, all the information I gleaned made sense, especially in light of the fact that traditional approaches (after MUCH expense on the part of Wausau AND us!) had not worked.
You can see from the enclosed information that Dr. Greenberg and the Magaziner Center for Wellness have very impressive credentials and certainly a very impressive track record of healing! For the sake of brevity in this letter, I am enclosing pertinent information regarding prolotherapy. You have to know that after having experienced 3 ½ years of truly debilitating pain and so many traditional attempts to find resolution, I am NOW pain free after having prolotherapy administered by Dr. Greenberg.
For someone who could not (before prolotherapy administration by Dr. Greenberg) ever sleep more than two hours through the night without having to waken with pain, could not lift her grandchildren, could not sit OR stand OR lie down without pain, could not enjoy the simple pleasures of LIFE, I submit to you that I am now PAIN FREE!! I am not only pain free, but due to the dynamics of prolotherapy, my involved ligaments and tendons are healed! I began my prolotherapy sessions with Dr. Greenberg on June13, 2005 and returned to his office at 3 week intervals. Each appointment, I was examined, assessed and given prolotherapy injections based on the results of his examination. As you are aware, I paid these charges out of my own pocket and submitted the expense form to Wausau. Dr. Greenberg is also out of network for me so my reimbursement would be only 65% versus the traditional 85% reimbursement I would get if I were in network. What a difference from a one-time, one injection charge of $883.36 that Wausau paid on March 9, 2005! With no result!!!
From the first appointment that I received the prolotherapy injections, I began to experience relief! Ensuing appointments continued to produce relief to the point where I no longer have the pain. While sitting in the waiting room at Dr. Greenberg's practice, I heard time and time again, testimonials to pain relief and resolution after prolotherapy. I challenge you to review the enclosed information. I am now healed, NOT pain managed!!!
I challenge you, (with an open mind) to assess the successful pain resolution and the method used to achieve it. As you will read in the enclosed information, prolotherapy has been used since the time of Hippocrates and with much success. Also, you will read that the former Surgeon General, C. Everett Koop, was healed of debilitating pain with prolotherapy and was so impressed with prolotherapy that he began treating scores of his own patients with prolotherapy. Why would I not choose the prolotherapy route after other approaches failed!?! Quality of life was the deciding factor! I will always be thankful to Dr. Greenberg for his skills and devotion to a non-traditional approach that has truly given me and so many others a quality of life again.
Once again, I can now participate in life events as a wife, mother and grandmother. I look forward to returning to nursing again as well! I submit to you this compilation of materials and trust that you will consider compensating at 65% ALL my claims that were not paid at that percentage. Please consider as well that I conscientiously made the decision to have prolotherapy and not subject myself OR my insurance company (Wausau) to further unjustifiable expense just falling through the cracks in the health care system, i.e.: continuing to go to PAIN MANAGEMENT!
